چکیده
Purpose: Surgical resection in one stage of primary colon cancer and synchronous liver metastases is an acceptable option. However, it causes significant surgical stress with high perioperative morbidity. The use minimally invasive surgery techniques could ameliorate this effect. We report a case simultaneous laparoscopic sigmoidectomy left hepatectomy using ICG fluorescence-guided techniques. Method: 86 years old female patient was referred to our unit for management colorectal located the sigmoid single hepatic metastasis measuring 6 cm between middle vein (seg. IVa). underwent neoadjuvant treatment six cycles FOLFOX, which led response (max. diameter reduced 4 cm). A decided. Material: 24 hours before surgery, 5 mg iv injected into patient. Firstly after introducing first trocar pneumoperitoneum complete mesocolic excision end anastomosis circular stapler performed. After that, visualized by switching mode tumour identified. Hilar dissection performed artery, portal were found ligated hemo lock. demarcation line optical imaging system injecting 2,5 iv. Liver transection Cusa, harmonic scalpel, intermittent Pringle manoeuvre fifteen min on five off. duct also confirmed under guidance stapler. Both specimens removed through Pfannenstiel incision. Total operation time 3 hours, estimated blood loss 300cc. postoperative course uneventful, discharged 5th day. Conclusion: Simultaneous colectomy major are challenging operations that should be only experienced centres can reduce helps locate tumour, correct line, biliary structures vascular anastomotic perfusion bowel anastomosis.

Simultaneous left colectomy and standard hepatectomy reformed by laparoscopy.
Laparoscopic approaches have been increasingly used in patients with colorectal or liver cancer. Simultaneous colectomy and hepatectomy are considered safe techniques and present similar oncological results regardless of the location of the primary tumor when there are fewer than four liver metastases, since there is no increase in morbidity or decrease in survival. The development of laparosco...
متن کاملSurgical outcomes following laparoscopic major hepatectomy for various liver diseases
The aim of the study was to report surgical outcomes (efficacy and safety) of laparoscopic major hepatectomy for various liver diseases.Although the number of laparoscopic liver resections has increased, expansion of laparoscopic major hepatic resection remains limited, mainly owing to the technical difficulties for the procedure as compared to open surgery. We describe our experiences with lap...

PURPOSE To evaluate how information from combined coronal optical coherence tomography (OCT) and confocal laser scanning ophthalmoscopy (SLO) with integrated simultaneous indocyanine green (ICG) dye angiography can be used in the diagnosis of a variety of macular diseases. METHODS A compact chin-rest-based OCT/confocal imaging system was used to produce the OCT image and excite the fluorescen...

BACKGROUND To compare short-term and long-term results of colorectal patients undergoing laparoscopic and open hepatectomy. Moreover, outcomes of laparoscopic versus open procedures for simultaneous primary colorectal tumor and liver metastasis resection were compared. METHODS A systematic search was conducted in the PubMed and EmBase databases (until Oct. 22. 2013) with no limits. Bibliograp...
متن کاملLaparoscopic Hepatectomy is Easier in Repeat Hepatectomy
Hepatic malignancies are frequently associated with primary liver disease, including hepatitis and cirrhosis. It is not uncommon for selected patients to require repeated operations for tumor recurrence. Subsequent operations are more challenging because of dense adhesions from prior operations combined with the inherent risk for liver dysfunction. We present the case of a 64-year-old man with ...
